What is the procedures for dealing with SLA( Service Level Agreement ) Failure

Depends what you mean as a "failure". SLA's are to be evaluated periodically. the setup of the SLA must mathc the feasibility of the serivice you wanna provide otherwise you are doomed to failure. Mostly you only some parameters exceed set values. At that moment corrective actions lust be taken on the level of resources, metric collection, sla setting, quality, procedures etc. Without knowing what its impossible to give advice.
